Sunny Day Behavioral ("SDB," "we," "our," or "us") is committed to honoring and upholding the rights of every client while establishing clear expectations for participation in our services. This document defines the fundamental rights and responsibilities of clients and/or their authorized representatives in the context of Applied Behavior Analysis (ABA) and related services.
Client Rights
Clients receiving services through Sunny Day Behavioral are entitled to:
Respect & Dignity
Be treated with compassion, courtesy, and without discrimination based on race, religion, sex, national origin, disability, age, gender identity, sexual orientation, or socioeconomic status.
Confidentiality
Expect all protected health information (PHI) and personal data to be maintained in compliance with HIPAA and applicable privacy laws.
Informed Consent
Receive clear, understandable information about services, goals, risks, benefits, alternatives, and your right to decline or withdraw at any time.
Participation in Care
Actively contribute to treatment decisions, including goal setting, program implementation, and behavior planning.
Access to Records
Review your own clinical records and request amendments in accordance with federal and state law.
Safety & Freedom from Harm
Receive services in a safe, non-threatening environment free from abuse, neglect, exploitation, or corporal punishment.
Cultural & Linguistic Respect
Request services delivered in your preferred language or with culturally responsive accommodations.
Timely Service
Receive sessions according to the agreed treatment plan and be notified of cancellations or changes.
Grievance Procedure
Submit complaints or concerns without fear of retaliation and receive timely acknowledgment and resolution.
Right to Refuse
Decline specific procedures or treatment recommendations and be informed of any consequences associated with refusal.
Freedom of Choice
Select or change providers (as available) and terminate services at your discretion.
Client Responsibilities
To support optimal outcomes and maintain a collaborative therapeutic relationship, clients (or their guardians) are expected to:
Provide Accurate Information
Disclose complete and truthful details about the client’s needs, background, behaviors, and medical or educational history.
Participate Actively
Be involved in treatment planning, parent training, and generalization of learned skills across settings.
Maintain Attendance
Commit to scheduled appointments and notify us of cancellations with at least 24 hours’ notice.
Respect Staff and Environment
Interact respectfully with SDB staff and avoid aggressive, disruptive, or disrespectful behavior.
Observe Safety Protocols
Follow all clinic, in-home, or school-based safety procedures, including masking, supervision, or other guidelines.
Collaborate with Providers
Share concerns, updates, and progress to help providers tailor services to evolving needs.
Protect Confidentiality
Refrain from recording sessions without prior written consent and avoid sharing other clients’ private information.
Adhere to Financial Agreements
Fulfill financial obligations, including insurance authorizations, co-pays, and any self-pay balances.
Report Issues Promptly
Notify us of changes in insurance, contact information, custodial rights, or household status that may impact care.
Follow Ethical Guidelines
Uphold ethical standards in communication, participation, and documentation related to SDB services.
Acknowledgment
By participating in services at Sunny Day Behavioral, you acknowledge that you understand and agree to uphold the rights and responsibilities outlined in this document.
